Our library includes some of the many different written asthma action plans available.

The asthma action plan chosen should be appropriate for the person’s age, educational status, language and culture.

Our template is below, along with links to alternative options.

National Asthma Council Australia

Revised and updated in 2011 by the National Asthma Council Australia in consultation with leading respiratory clinicians. Development was supported by GlaxoSmithKline Australia and National Asthma Council Australia retained editorial control.

Electronic PDF versions are available in full colour, black and white or a writable PDF. An RTF version is also available.


Note: We are working towards inclusion of the asthma action plan in medical prescribing software programs. In the meantime, you may wish to save an electronic version of the action plan in your practice's prescribing software. The PDF version can be saved as a patient education resource. The RTF version can be saved as a letter writer template, which may then need amending to suit your practice’s administrative processes.

 

Asthma Cycle of Care Asthma Action Plan

Developed with the National Asthma Council Australia and Asthma Foundations Australia by the Australian Government Department of Health and Ageing as part of the Asthma Cycle of Care 2007.

Symbicort SMART Asthma Action Plans

This asthma action plan is specifically for patients using the combination medication Symbicort (budesonide and eformoterol) maintenance and reliever therapy (SMART). Note Symbicort is not recommended for use in Australia in children under 12 years.

The Symbicort SMART Asthma Action Plans for Rapihaler and Turbuhaler were independently developed by clinicians. Hard copies (tear-off pads) can be ordered from AstraZeneca via local representatives or by calling 1800 805 342. 

The SMART asthma action plans is also available electronically on the medical prescribing software Medical Director.

Asthma Action Plans for Indigenous Australians

Remote Indigenous Australian Asthma Action Plan

This asthma action plan was developed specifically for Indigenous peoples with asthma who live in remote areas of Australia.

The Remote Indigenous Australian Asthma Action Plan was developed by the Australian Department of Health and Ageing based on a plan designed by Professor Anne Chang. The imagery is from the Short Wind educational materials distributed by the Asthma Foundation of the Northern Territory. This asthma action plan was funded by the Australian Government.

Remote Indigenous Australian Asthma Action Plan

 

Every Day Asthma Action Plan

This asthma action plan was developed specifically for remote Indigenous Australians with asthma who do not have a high level of literacy in English. It may also be useful for other people with asthma who are not comfortable with complex written English.

The Every Day Asthma Action Plan was developed by the Spencer Gulf Rural Health School, Pika Wiya Health Service and Asthma SA as part of an Indigenous Asthma Innovative Management (AIM) project, supported by the Department of Health and Ageing.

Paediatric Asthma Action Plans

Two asthma action plans for children have been developed by the Sydney Children's Hospital, Randwick.

For parents

Designed for parents and carers of children with asthma, this text-based asthma action plan includes information on what to look for in an asthma attack and how to recognise when symptoms are getting better.
