There are many different written asthma action plans available. The asthma action plan chosen should be appropriate for the person’s age, educational status, language and culture.
The current NAC template is given below, along with links to alternative options.

Symbicort SMART Asthma Action Plan |

This asthma action plan is specifically for patients using the combination medication Symbicort (budesonide and eformoterol) maintenance and reliever therapy (SMART).
Note Symbicort is not recommended for use in Australia in children under 12 years.
The Symbicort SMART Asthma Action Plan was independently developed by clinicians.

Remote Indigenous Australian Asthma Action Plan
This asthma action plan was developed specifically for Indigenous peoples with asthma who live in remote areas of Australia.
The Remote Indigenous Australian Asthma Action Plan was developed by the Australian Department of Health and Ageing based on a plan designed by Professor Anne Chang. The imagery is from the Short Wind educational materials distributed by the Asthma Foundation of the Northern Territory. This asthma action plan was funded by the Australian Government.

Every Day Asthma Action Plan
This asthma action plan was developed specifically for remote Indigenous Australians with asthma who do not have a high level of literacy in English.
It may also be useful for other people with asthma who are not comfortable with complex written English.
The Every Day Asthma Action Plan was developed by the Spencer Gulf Rural Health School, Pika Wiya Health Service and Asthma SA as part of an Indigenous Asthma Innovative Management (AIM) project, supported by the Department of Health and Ageing.

Resources in community languages
Asthma action plans are not readily available in Australia in languages other than English.
The NAC is interested in developing Australian asthma action plan templates for people with asthma from culturally and linguistically diverse communities.
