An asthma action plan needs to be easily understood by the person with asthma or their carer.

Many written asthma action plans are available, as different action plans suit different people. You and your doctor should choose a template that you can understand easily and that suits your circumstances.

National Asthma Council Australia Asthma Action Plan

Symbicort SMART Asthma Action Plan

This asthma action plan is specifically for patients using the combination medication Symbicort (budesonide and eformoterol) maintenance and reliever therapy (SMART). Note Symbicort is not recommended for use in Australia in children under 12 years.

The Symbicort SMART Asthma Action Plan was independently developed by clinicians. Hard copies (tear-off pads) can be ordered from AstraZeneca via local representatives or by calling 1800 805 342.
