Our library includes some of the many different written asthma action plans available.
The asthma action plan chosen should be appropriate for the person’s age, treatment regimen, asthma severity, culture, language, literacy level, and ability to self-manage.
Our template is below, along with links to alternative options. Please contact us if you would like your organisation’s asthma action plan considered for inclusion in this section.

Medical prescribing software
We are working towards inclusion of the asthma action plan in medical prescribing software programs. In the meantime, you may wish to save an electronic version of the action plan in your practice's prescribing software. The PDF version can be saved as a patient education resource. The RTF version can be saved as a letter writer template, which may then need amending to suit your practice’s administrative processes.

Fostair® asthma action plan
This asthma action plan is specifically for patients using the combination medication Fostair® (beclometasone and formoterol) as a maintenance and reliever therapy.
Note: Fostair® is approved for use by adults 18 years and older in Australia

Symbicort anti-inflammatory reliever +/- maintenance action plans
This asthma action plan is specifically for patients using the combination medication Symbicort (budesonide and eformoterol) as an anti-inflammatory reliever with or without maintenance.
Note: Symbicort is not recommended for use in Australia in children under 12 years.
The Symbicort asthma action plans were independently developed by clinicians. Hard copies can be ordered from AstraZeneca via local representatives or by calling 1800 805 342.
These are symptom based asthma action plans. Copies of the peak flow based action plan are available from AstraZeneca.
Asthma action plans for Indigenous Australians
Remote Indigenous Australian Short Wind Asthma Action Plan
This asthma action plan was developed specifically for Indigenous peoples with asthma who live in remote areas of Australia.
The Remote Indigenous Australian Asthma Action Plan was developed by the Australian Department of Health based on a plan designed by Professor Anne Chang. The imagery is from the Short Wind educational materials distributed by the Asthma Foundation of the Northern Territory. This asthma action plan was funded by the Australian Government.
Victorian schools asthma action plans
Asthma Australia, in consultation with the Victorian Department of Education, catholic and independent schools, has developed children's service and school specific Asthma Action Plans. This meets the needs of the Victorian Education and Training Reform Act 2006, the Education and Training Reform Regulations 2007 and the Schools Policy Advisory Guide.
These plans complement the child’s Asthma Management Plan, whether it be NAC’s Asthma Action Plan, or Asthma Australia’s Asthma Care Plan in an Education and Care Setting.
The Victoria Asthma Action Plans assist staff members to identify students asthma signs and symptoms including their severity and what action needs to be taken regarding administering asthma reliever medication.
Asthma care plan for education and care services
Developed by Asthma Australia, the Asthma Care Plan should be completed by the treating doctor and parent/guardian, for supervising staff and emergency medical personnel.
